KJN Therapy Consumer Health Data Privacy Policy

This policy explains how KJN Therapy PLLC ("we" or "us") collects, uses, and shares consumer health data under Washington's My Health My Data Act (RCW 19.373). It applies to information collected through kjntherapy.com and from people who contact us before becoming clients.

Information in client records is protected under HIPAA and Washington's health care information law. It is covered by our Notice of Privacy Practices, not this policy.

Consumer health data we collect and why

We collect only what we need to respond to you and run our website.

  • What you send us. When you use our contact or consultation form, email us, or call us, we collect your name, contact information, and anything you choose to share about why you're seeking therapy. We use this to respond, schedule a consultation, and decide together whether we're a good fit.

  • Website usage information. When you visit kjntherapy.com, our website platform uses cookies to collect your IP address, device and browser type, and the pages you view. These analytics cookies are placed only if you accept them through our cookie banner. Because our site describes mental health services, this could suggest you're interested in therapy. We use it to operate, secure, and improve the site.

We won't collect other types of consumer health data, or use this data for other purposes, without first telling you and getting your consent.

Where the data comes from

  • Directly from you, through our forms, email, or phone.

  • Automatically from your device when you visit our website, if you accept analytics cookies.

Sharing

We do not sell consumer health data, and we do not share it with third parties or affiliates. We use service providers that handle data on our behalf, under contract, only to provide our services. These categories of providers are:

  • Website hosting

  • Email

  • Practice management and scheduling

  • Website analytics

Your rights

If you live in Washington, or your data was collected in Washington, you have the right to:

  • Confirm whether we collect, share, or sell your consumer health data, and get a copy of it. This includes a list of any third parties or affiliates we've shared it with and how to contact them.

  • Withdraw your consent to our collecting or sharing your consumer health data.

  • Have your consumer health data deleted.

How to make a request. Email kelly@kjntherapy.com with the subject line "Health Data Request" and tell us what you'd like us to do. We may ask for information to confirm your identity.We'll respond within 45 days. If we need more time, we'll tell you within that period and may extend it once by up to 45 days. Requests are free up to twice a year. If your data is in a backup system, deletion from that system may take up to six months. We won't treat you differently for exercising these rights.

Appeals

If we decline your request, you can appeal by emailing kelly@kjntherapy.com with the subject line "Health Data Appeal." We'll respond in writing within 45 days, explaining what we did or didn't do and why.

If we deny your appeal, you can file a complaint with the Washington State Attorney General's Office at www.atg.wa.gov.
